Social Prescribing Link Worker
Help on Your Doorstep is a community-based Islington charity which aims to improve individual and community wellbeing. Help on Your Doorstep seeks to empower residents and communities through provision of information, advice and guidance, referral and navigation of services and provision of medium term 1 to 1 support.
Help on Your Doorsteps Social Prescribing service supports patients referred by GP practices in the North and Central areas of Islington. The service benefits from working closely with the Connect service and access to over 140 referral partners covering a wide range of needs and aspirations. Social Prescribing is a fast paced, patient focused environment where patients referred are supported to link into services and activities which will improve their health and wellbeing by addressing the wider determinants of health.
We are looking for candidates who can actively listen with excellent interpersonal skills and experience of providing personalised support, focusing on what matters to the patient/client, to a varied caseload. The ideal candidate will have knowledge of local services and be passionate about supporting people to improve their wellbeing and maximise their potential.
A methodical and organised approach to managing a caseload and completing related administrative tasks is essential. Experience of working with NHS partners within Social Prescribing or otherwise is advantageous but non-essential.
Successful applicants will also need to undertake a package of mandatory training modules as part of induction.

Connect Advisor - Outreach, Referrals and Community Navigation
Our team of Connect Advisors play an important role supporting residents in Islington by providing direct support to residents by signposting them to services, providing advocacy and support with form filling. We bring services and support right to people's doorstep.
We work at the heart of the voluntary sector and have a long-established network of partners that we work alongside to deliver support to residents in Islington. Our work with partners is all about building relationships that break down silos and facilitate co-production and shared learning.
If you are a proactive self-starter with a passion to empower people experiencing challenges in Islington, then this Connect Advisor role at Help on Your Doorstep could be the role for you!
