About us Work for us Work with us Social Prescribing Link Worker We are looking for a Social Prescribing Link Worker to join our established team. Our Social Prescribing service supports patients referred by GP practices in the North and Central areas of Islington. Role overview Social prescribing link workers will work with the Help on Your Doorstep Connect team and the Primary Care Network (PCN) multi- disciplinary team. Social prescribing will support PCNs to strengthen community and personal resilience, reduce health inequalities (in relation to timely access and outcomes) and wellbeing inequalities by addressing the wider determinants of health. The postholder will work with people with long term conditions (including mental health), people who are lonely or isolated, or have complex social needs which affect their wellbeing. Location: Help on Your Doorstep's offices in Central and North Islington Hours: 35 hours per week or 17.5 hours per week (please state your preference for full-time or part-time in your application) Salary: £28,270 pro rata per annum plus contributory pension For further information please see the job description and person specification.
