Current vacancies Social Prescribing Service Team Manager (full-time, permanent) About the role The Social Prescribing Service Team Manager will ensure the effective and efficient delivery of all Social Prescribing services; team leadership, service development and play an integral role in the wider organisational leadership to foster an inclusive and collaborative team culture amongst social prescribers and other existing teams. Role Purpose: To manage and oversee the successful delivery of our primary care social prescribing services. providing management, leadership and support to a team of practitioners based on a broad understanding of evidence-based practice and expertise in working with people with complex levels of need that are preventing them from effectively addressing their physical and mental health. Manage a small caseload delivering high quality 1 to 1 services to meet the needs of adults and local communities. Key Responsibilities: To work with Islington GP federation and the Clinical Director in each PCN to deliver a coordinated and high quality Social Prescribing Link Worker (SPLW) service, supporting residents to access and engage with the extensive range of support in the community. Lead, develop, and supervise a team of SPLWs to deliver a high quality service Support the development of Social Prescribing in Islington and represent and promote the service to stakeholders Working with the Head of Service to help to develop the service, oversee operational improvements and participate in support, supervision and training as required. Social Prescribing Link Worker (part-time, permanent) About the role Social prescribing link workers will work with the Help on Your Doorstep Connect team and the Primary Care Network (PCN) multi- disciplinary team. Social prescribing will support PCNs to strengthen community and personal resilience, reduce health inequalities (in relation to timely access and outcomes) and wellbeing inequalities by addressing the wider determinants of health. The post will work with people with long term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.
