Our Social Prescribing service supports patients referred by GP practices in the North and Central areas of Islington. Our Social Prescribing service benefits from working closely with our Connect service and we have access to over 120 referral partners covering a wide range of needs and aspirations. Social Prescribing is a fast paced, patient focused environment where patients are referred by GPs so we can link them into services and activities which will improve their health and wellbeing. It is an exciting time to join Help on Your Doorstep as we are embarking on further development of our services, giving an opportunity to help shape the future of service delivery. We are looking for candidates with excellent interpersonal skills and experience of providing personalised support, focusing on what matters to the patient/client, and managing a varied caseload. The ideal candidate will have knowledge of local services and be passionate about supporting people to improve their wellbeing and maximise their potential. A methodical and organised approach to managing a caseload and completing related administrative tasks is essential. Experience of working with NHS partners within Social Prescribing or otherwise is advantageous but non-essential.
