Social Prescribing Service Team Manager (full-time, permanent - 35 hours per week, working 21 hours as a manager and 14 hours social prescribing caseload)

The Social Prescribing Service Team Manager will ensure the effective and efficient delivery of all Social Prescribing services; team leadership, service development and play an integral role in the wider organisational leadership to foster an inclusive and collaborative team culture amongst social prescribers and other existing teams.

Social Prescribing Link Worker (part-time, permanent)

About the role

Social prescribing link workers will work with the Help on Your Doorstep Connect team and the Primary Care Network (PCN) multi- disciplinary team. Social prescribing will support PCNs to strengthen community and personal resilience, reduce health inequalities (in relation to timely access and outcomes) and wellbeing inequalities by addressing the
wider determinants of health. The post will work with people with long term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.
